The placebo effect The placebo effect occurs when patients experience genuine improvement in their symptoms after receiving a treatment that has no active medical ingredient — a sugar pill, a saline injection, or even a sham surgical procedure. The effect is not imaginary: brain imaging studies show that placebos can trigger measurable changes in brain chemistry, including the release of endorphins and dopamine in pain-related circuits. The size of the effect varies depending on several factors. Elaborate placebos work better than simple ones: an injection is more effective than a pill, and a large pill is more effective than a small one. The doctor-patient relationship also matters: patients who believe their doctor is confident and caring respond more strongly. How the gut affects the brain The human gut contains roughly one hundred trillion microorganisms that collectively weigh about two kilograms. This community, known as the gut microbiome, does far more than digest food. Research over the past decade has revealed a complex communication network between the gut and the brain, called the gut-brain axis. Gut bacteria produce neurotransmitters, including about ninety per cent of the body's serotonin, which regulates mood, sleep, and appetite. The vagus nerve, a long nerve connecting the gut to the brain, carries signals in both directions, meaning gut health can directly influence mental health and vice versa. Animal studies have shown that transferring gut bacteria from anxious mice to calm mice makes the calm mice behave anxiously, and human studies have found associations between certain gut bacteria profiles and conditions like depression and anxiety. This has led to interest in psychobiotics — probiotics specifically designed to improve mental health — though the field is still in its early stages and much remains to be understood.

Personalised medicine and genomics Traditional medicine treats diseases based on symptoms and population-level evidence: if a drug works for most patients with a given condition, it is prescribed to all of them. Personalised medicine aims to tailor treatment to the individual patient's genetic profile, lifestyle, and environment. The Human Genome Project, completed in two thousand and three, laid the groundwork by mapping the entire human genetic code. Since then, the cost of sequencing an individual's genome has fallen from billions of dollars to under a thousand, making it feasible as a clinical tool. In cancer treatment, personalised medicine is already making a difference. Genetic analysis of a tumour can reveal which mutations are driving its growth, allowing doctors to select drugs that target those specific mutations rather than using broad-spectrum chemotherapy that damages healthy cells. Pharmacogenomics, a related field, studies how genetic variation affects an individual's response to drugs, helping doctors choose the right medication and dose while avoiding adverse reactions that affect genetically susceptible patients.
